Laboratory
Safety Rules and Contract
Dr. Wexler, Bay View High School
Read the following laboratory safety rules. You and your
parent/guardian must then sign the contract and return it to your instructor
in order for you to participate in laboratory activities.
The laboratory is a safe place to experiment if you are
careful. You must assume responsibility for the safety of yourself and your
neighbors. Here are some safety rules that will be consistently enforced in
the laboratory.
1. Do not perform experiments that are unauthorized. Always
obtain your teacher's permission.
2. Eye protection for certain types of experiments in the laboratory is essential
and should be worn at all times.
3. Eating and drinking in the laboratory are strictly forbidden.
4. Wear shoes that protect your feet. Sandals and many styles of open-toed shoes
are not much better than bare feet and are forbidden.
5. A laboratory jacket should be worn for certain lab activities.
6. Know the location of the fire extinguisher, eye wash station, and fire blanket.
7. Report any accident, injury, or dangerous activity to your instructor at
once.
8. Clean up spills (solid and liquid) at once.
9. Never engage in horseplay in the laboratory.
10 . Never taste any chemical substance unless you are directed specifically
by your teacher to do so.
11 . If clothing should catch fire, smother it with a blanket.
12 . Handle chemicals carefully. Check the label of all bottles before removing
the contents. Do not return unused chemicals to reagent bottles. Wear protective
gloves.
13. Keep combustible materials away from open flames. (Some of these materials
are alcohol, acetone, hexane.)
14. When heating a substance in a test tube, be careful not to point the mouth
of the test tube at yourself or another person. NEVER heat a stoppered test
tube or other closed container.
15. Keep solid waste material out of sinks.
16. Long hair should be tied back.
17. If you are not familiar with a piece of apparatus, ask your instructor for
help.
18. You are responsible for your lab station during
the entire lab period. If any equipment is missing or broken, tell your instructor
immediately. If you are out of a chemical or missing equipment DO NOT take it
from another lab station. Tell your instructor and he provide you with a replacement.
19. When you report to the lab area, check to make
sure that the lab group before has not left broken equipment, has cleaned up
the area, and things are generally in order so that you are not held responsible
for their mistakes.
20. The instructor may have a student immediately
removed from the laboratory setting if the student's actions pose a direct threat
to the safety and security of those around him/her.
22. All safety violations will be dealt with as follows:
1st offense: After-school detention to
review safety rules. Failure to appear results in a referral and parent/teacher
conference.
2nd offense: Parent/teacher conference.
3rd offense: Referral written
for administrative detention or suspension
4th offense: Referral written for transfer out of class.
I have read, understand and agree to the terms of this safety contract.
